Community Health Assessment - Community Health Improvement Plan

2022-2027 Community Health Assessment Process

The Bourbon County Health Department, in collaboration with researchers from the University of Kentucky Public Health, led a group of stakeholders in conducting a Community Health Assessment (CHA) using a modified Mobilizing for Action through Planning and Partnerships (MAPP) framework. From June to September 2022, we organized two forums and a community health survey to gather insights into the health and well-being of Bourbon County residents.

Throughout this process, we gathered and examined data to identify the current health outcomes impacting the residents of Bourbon County. The findings from the completed assessments revealed that mental health, substance use, and basic needs were the primary health concerns for both community organizations and residents.

Community Health Improvement Plan

In late 2022, Bourbon County completed its review of the Community Health Assessment (CHA) and developed a plan for improvement. The Community Health Improvement Plan (CHIP), created following the CHA process, serves as a roadmap outlining actions to enhance the community’s well-being based on the findings from the CHA assessment.

The goals, objectives, and interventions were developed with contributions from various stakeholders, focusing on enhancing specific health outcomes linked to the top three “health concerns” identified in the Community Health Assessment (CHA). The Bourbon County CHIP for the years 2022-2027 targets the following key areas of focus:

    • Housing
    • Mental Health
    • Alcohol, Tobacco, and Other Drugs
